This form is used to initiate a call back to schedule our FREE local pick-up within a 25 mile radius from Central Avenue and Camelback Street in Phoenix. Should you live beyond the 25 mile radius FREE area, we still come by and pick up and deliver, but in all fairness, we would have to apply a surcharge of $10.00 for time and driving expenses. Should you prefer to ship your material to AMS, you can use any size cardboard box print this form and enclose it in your USPS, UPS or FedEx shipment.
AMS will pay for return shipping expenses of your material and your new DVD(s) or flash drive(s) back to you after the job is done! No matter which way you go...you can't go wrong and you will be 100% satisfied - we guarantee it!
